• A 5 month’s male child admitted with c/o respiratory distress and cough.
• X-Rays chest s/o hyper lucency rt. Lung field
• CECT chest s/ort. congenital Lobar emphysema
• Operation - Rt. Thoracotomy at 4th ICS,
• Middle and lower lobectomy is done with secure the pulmonary vessels and trachea

History & Investigation

• A 45-years male presented in emergency department with chief complaints of dyspnea, fever and chest pain for the last 10 days on 15 July 2021.
• —History DVT right lower limb 6 month back.
• CT pulmonary angiography s/o – Pulmonary Thromoembolism bilateral Pulmonary Arteries and segmental branches.
• CT chest s/o ground glass haziness.
• 2DECHO finding – multiple thrombus in RA, RV with mod. TR, SEV RV dysfunction and normal LV function with EF 55%. 

Operation -midline sternotomy

• —Fullyheperisation.
• —On CPB
• —MPA, RPA and LPA opened- large clots adherant to wall, branches and right atrium are completely removed.
• —Smoothly off CPB, and finally closer patient shift to ICU.
• Post operative - managed through invasive monitoring and ventilator support, higher antibiotic and anticoagulant drugs are given with titrated doses.
• —Patient discharged with uneventful post-operative course.

Open Heart Surgery by Right THORACOTOMY Approach.ASD/VSD/DVR/AVR/MVR

• —A 41 years female admitted with chief complaint of dyspnoea on exertion, chest pain, lethargy and occasionally palpitation.
• —X-Ray chest s/o Cardiomegaly CT ratio>.55 and prominent broncho-vascular marking
• —2DECHO s/o ACCHD/situs solitus/Levocardia and large ASD secondum type.
• Operation - Rt. Thoracotomy , 4th ICS.
• —ASD is repaired with DVD patch.
• —Post operation : uneventful course and discharge.
